Patient Management Frameworks
The Cancer Services Framework for Victoria recommended that tumour streams be developed to reduce unwanted variation in practice.
Patient Management Frameworks (PMFs) were developed to provide a consistent statewide approach to care management in each tumour stream and are intended to improve patient outcomes. There are 15 PMFs available:
Osteosarcoma and soft tissue sarcoma
Breast tumour stream: breast cancer
Colorectal tumour stream: colon and rectal cancer
Gynaecological tumour stream: ovarian cancer
Head and neck tumour stream: larynx, pharynx and oral cancer
Lung tumour stream: non-small cell lung cancer
Skin tumour stream: melanoma
Genitourinary tumour stream: prostate cancer
Genitourinary tumour stream: testicular cancer
Upper gastrointestinal tumour stream: pancreatic cancer
Upper gastrointestinal tumour stream: oesophagogastric cancer
Central nervous system tumour stream: malignant glioma
Central nervous system tumour stream: cerebral metastases
Haematological tumour stream: acute myeloid leukaemia
Haematological tumour stream: intermediate grade non-Hodgkin lymphoma
Patient Management Frameworks - Toolkit
PMFs provide:
- A clear description of the care pathway, identifying the critical points along that pathway and the optimal model of care.
- Set out the key requirements for the provision of optimal care.
- Guide the patient journey to ensure patients with cancer and their families receive optimal care and support.
The PMFs have been developed in collaboration with a wide range of practitioners, consumers and carers. Wherever possible, they are based on current best practice, including clinical guidelines, care pathways, standards and research. In many cases, however, they are a statement of consensus regarding currently accepted approaches to treatment.
As a guide, the PMFs are to be followed subject to the health professionals independent medical judgment and the patients preference in each individual case.
