Serious Transfusion Incident Reporting (STIR) system pilot 2006

Summary

The Better Safer Transfusion (BeST) Serious Transfusion Incident Reporting (STIR) system was piloted during July - October 2006 to capture serious hospital transfusion incidents, including near misses. The data has been collated and reported with recommendations for improvements for better, safer transfusion practice.

The system reports on incidents relating to fresh blood and blood products, namely red cells, fresh frozen plasma, platelets and cryoprecipitate. This includes products from volunteer donors, family donors and autologous collections.
