Archived August 20 2008 - may contain information of historical interest (some links may not work)Risk Screening Tool - OverviewIn 1997/1998, the Department of Human Services commissioned the development, pilot and validation of a uniform risk screening tool for potential application across all hospitals. The risk screening tool was developed by Shane Thomas and Associates Pty Ltd in collaboration with PAC projects. It was designed to identify people at potential risk of poor health outcomes on discharge by identifying those patients who might require a more detailed assessment of post acute care requirements. The tool utilises a range of clinical, demographic and social factors which predict post acute care need, and forms the basis of the performance indicators for effective discharge. The project was completed in August 1998.
