Diabetes resource workers in Goulburn Valley General Practice

The Integrated Diabetes Care Program is a three year project funded by the Victorian Department of Human Services under their Quality Improvement and Best Practice Funding Program of 2001. The project has three broad aims - improve workforce capacity for diabetes care, promote diabetes self management and integrate diabetes care.
You are welcome to view the submission document or discuss the progress of the project with individual steering committee members.

Member agencies from the Shepparton and Moira Shires include

  • Numurkah District Health Service
  • Nathalia District Hospital
  • Goulburn Valley Health
  • Cobram District Hospital
  • Yarrawonga District Health Service

Steering Committee Members

The Program is supported by an enthusiastic steering committee, comprising a representative from each of the member agencies and the Goulburn Valley Division of General Practice. Helen Davis (Numurkah), Deb Barnes (Nathalia), Gloria Kilmartin (Goulburn Valley), Leigh Gibson (Goulburn Valley), Jeannie Florence (Cobram), Sally Stapleton (Yarrawonga), Dr David Kelly and Deb Cottrell (GVGP). The steering committee meets each month to discuss the progress of the program, and the timelines set for the program. The program has close ties with the Goulburn Valley PCP.

The project evaluation team includes Faye Hosie (Master Rural Health candidate), Associate Professor Bridget Hsu-Hage and Carole Maddison from the School of rural Health, University of Melbourne in Shepparton.
