The Department of Human Services committed $150 million to HARP over the four year period to June 2005. The presentations from the “HARP – The Future” Workshop provide a summary of HARP’s achievements and the future directions for the Victorian Chronic and Complex Care Program. We are moving from a focus on ‘innovation’ and ‘experimentation’ to embedding successful models of care within the service system. To support this transition, the Department of Human Services has identified key elements of successful models of care from which the Victorian Chronic and Complex Care Program Guidelines have been developed.
