Welcome to the Hospital Admission Risk Program (HARP)

Better health for Victorians: Reducing the avoidable use of hospitals

Our vision is of better health for Victorians through preventing the avoidable use of hospitals.

HARP has done this by:

  • Developing preventive models of care that involve both the hospital and the community,
  • Focusing on people who have a manifest health need, often where their disease or condition is chronic or complex, and
  • Giving priority to high volume and/or frequent users of the acute public hospital system.

The Department of Human Services committed $150 million to HARP over the four year period to June 2005.

The presentations from the “HARP – The Future” Workshop provide a summary of HARP’s achievements and the future directions for the Victorian Chronic and Complex Care Program. We are moving from a focus on ‘innovation’ and ‘experimentation’ to embedding successful models of care within the service system. To support this transition, the Department of Human Services has identified key elements of successful models of care from which the Victorian Chronic and Complex Care Program Guidelines have been developed.
