Chronic Heart Failure Disease Management Working PartyIt is estimated that Chronic Heart Failure (CHF) affects at least 300,000 Australians, with approximately 30,000 new cases diagnosed annually. The number of people with this condition is expected to increase markedly due to the ageing population, the projected increase in the number of older people with coronary heart disease and hypertension, and the decrease in case fatality rates associated with acute coronary syndromes. There is an increasing body of evidence that supports the use of more holistic and integrated models of care, such as Disease Management programs, for people with CHF. The key focus of this Working Party is to establish a systematic framework for managing the CHF patient population that:
