Integrated Care for Complex Needs Working PartyMany elderly patients present complex medical and social challenges. They may suffer with "geriatric syndromes" such as cognitive impairment, poor mobility, falls, incontinence, and are at risk of experiencing functional decline during hospitalisations. This may contribute to longer lengths of stay, increased risk of complications, iatrogenic events, impairment of physical performance and additional need for sub-acute and/or residential care. The Integrated Care for Complex Needs Working Party will examine models for integrating care for frail, elderly people with complex needs, and determine where these models can be extended or duplicated in order to optimise care for this complex group.
