HARP Funded Projects

A commitment of $150 million over 4 years was made to HARP. These funds were allocated as follows:

  • 2001/02 - $17 million
  • 2002/03 - $33 million
  • 2003/04 - $50 million
  • 2004/05 - $50 million

2001/02 Funding

More than forty prevention initiatives were funded across a diverse range of projects, including:

  • Emergency Department Care Coordination
  • Disease Management models of care
  • Acute - Primary Care Liaison projects
  • Prevention Initiatives Targeting Seniors

2002/03 Funding

The projects initially funded in 2001/02 continued with a further 42 projects being funded which commenced in 2002/03. The importance of collaboration and service linkages between acute and primary care providers was emphasised throughout the funding round, resulting in significant resources flowing into primary care organisations.
