The Ambulatory Care Framework (ACF) builds on a number of key policies linked to the Metropolitan Health Strategy, specifically:

These publications are available for download from the Metropolitan Health Strategy site publications page .

The ACF is aligned with the development of the Hospital Admission Risk Program (HARP) into HARP Chronic Disease Management.

The ACF also builds on policy directions for primary health care in Victoria, including the Primary Care Partnership Strategy, and Community Health Services - creating a healthier Victoria.
