Care in your communityPage contents: Introduction | Download documents | Contact IntroductionCare in your community represents a significant change in how health care will be delivered in the future. It is a vehicle for building the capacity of the Victorian health care system to deliver person-centred health care in community settings, reducing the need for in-patient care, and improving the health outcomes of Victorians. It provides clear policy directions and an overarching planning framework for ambulatory care service delivery models and facilities in Victoria, building on established and successful elements of the current health system. Implementation will be undertaken during 2006 to test and refine the framework. A broader roll-out of integrated area-based planning and supporting elements is expected to commence in 2009. As part of the implementation work, CiYC was trialled across three sites with evaluation of the implementation methodology being done. This evaluation was completed by independent consultants and is available below. The consultants have submitted their final report following close consultation with CiYC governance structures. The trials of CiYC were successful; there was strong stakeholder engagement and consensus on a range of areas. The evaluation of the CiYC trials has highlighted the key areas for further attention to ensure effective roll-out of CiYC policy.
