Strengthening Care Outcomes for Residents with EvidencePage content: Overview | Download documents OverviewAs part of its Strengthening Care Outcomes for Residents with Evidence initiative (SCORE), the Department of Health commissioned the Australian Centre for Evidence Based Aged Care (ACEBAC) to develop and pilot evidence based standardised care processes (SCPs) to support Victorian Health Services operating aged care homes to provide high quality care to residents. The SCPs have been refined, taking into account available evidence and input from those sites engaged in the pilot during 2009. Copies of the final SCPs and the summary final evaluation report are available. Organisations wanting to translate evidence into practice using the standarised care processes in residential aged care services, need to consider:
