Strengthening Care Outcomes for Residents with EvidencePage content: Overview | Phases | Download documents Overview'SCORE': Strengthening Care Outcomes for Residents with Evidence is an initiative commissioned to support Victorian Health Services operating aged care homes to provide high quality care to residents. SCORE has a focus on managing some key areas of risk for residents and seeks to develop and implement evidence based standardised care processes. PhasesSCORE has two phases. Phase onePhase one consists of three main stages.
The Australian Centre for Evidence Based Aged Care (ACEBAC) is engaged to develop and implement phase one of SCORE. Professor Rhonda Nay, Director of ACEBAC, and her team, in consultation with the department, have been working with participating Health Services. Stage 1 has now been completed. The following ten areas of risk were prioritised and draft evidence based standardised care processes are developed for each risk.
Stage 2 has commenced and involves implementing up to four of the care processes in each of six pilot sites. Phase twoPhase two will involve the broader development and implementation of the care processes and tools across PSRACS. Download documents
