Overview

Well for Life successfully supported the capacity of Planned Activity Groups to encourage improved physical activity and nutrition with participants. There is alignment between the objectives and principles of Well for Life and the proposed quality enhancement in Home and Community Care (HACC) service delivery known as the Active Service Model.

The Victorian Minister for Aged Care made funding available to pilot Well for Life learnings from HACC Planned Activity Groups (PAGs) into other HACC services (home care, personal care, assessment and care management, property maintenance and delivered meals).  Councils that had previously implemented Well for Life in Phase 1 (2003-04) or Phase 2 (2004-05) were chosen to undertake pilot projects. These projects and their evaluation have concluded and an overview is provided below.

Murrindindi Shire Council, Hume Region
Look good, Feel good, eat well.

This project capitalized on collaborative working partnerships already established between the Shire, Mitchell Community Health Service and Berry Street Volunteer Resource Centre.  It involved the design and implementation of a time limited program. This ran on a weekly basis with a mix of education and physical activity sessions, following assessment and the development of personal goals.

Shire of Melton, Brimbank and Maribyrnong Councils and Victoria University, North & West Metropolitan Region
Active Clients Active Workers Project

The project is collaboration between the Councils of Melton, Brimbank and Maribyrnong, and Victoria University. The project trialled the roles and measured the benefits of using a) community care staff to support people to be involved in physical exercise; and b) exercise physiologists in working with HACC clients.
