Falls prevention checklist

Format: Self checklist for falls risk

This checklist was originally developed for residents in Supported Residential Services. It is a two - page checklist that enables residents to identify their own risk factors for falling. The accompanying two page falls prevention action sheet encourages residents to think about which falls prevention measures they can follow to reduce reduce their risk of falling.

Staff and families/friends could assist residents in completing the checklist and, if necessary, identify ing the most appropriate actions if necessary to reduce the number of falls and minimise the impact of fall-related injuries.
