Incident report protocol

Title: Protocol for completing incident forms related to falls

Developed by: National Ageing Research Institute

Format: Incident report protocol

This protocol provides a checklist of items that need to be performed when completing incident forms related to falls. It can be attached to the incident report folder as a reminder to all staff when they complete the form and/or displayed in the staff room.

It also has a list of other issues that should be considered when a fall occurs, such as when the last risk assessment was conducted and reviewed. In addition, the protocol explains what happens with the incident reports each month, and how they should be audited.
