How safe are you from falling?Produced by: Central Bayside Division of General Practice (based on the Stay on Your Feet - Western Australia checklist) Format: Information sheet (2 pages) This self-assessment checklist includes questions related to previous falls, medications, exercise levels, health conditions, balance and walking and vision. A yes response to any of the questions may indicate an increased risk of falling. Seniors should discuss the questionnaire with their doctor or health practitioner about how to reduce their risk of falling. The checklist was modified and translated into other languages by the Central Bayside Division of General Practice in a project funded by the Commonwealth Government Department of Health and Ageing. Copies of the English, Greek, Italian and Turkish versions of the checklist and other falls prevention information are downloadable from the Division's website. Copies of a similar checklist modified and translated into other languages by Stay on Your Feet - Adelaide west are available in Greek, Italian, Macedonian, Polish, Serbian, Spanish, Turkish and Vietnamese from this website. Note: In 1998, the Department of Health Western Australia began a five-year statewide falls prevention program called Stay on Your Feet WA. The program first developed the original checklist in 1999. The self checklist was modified in 2005 and is available from the Stay On Your WA website (external link). The checklist is available as a one-page handout or as part of a falls prevention brochure.
