Incident formsDetails of falls and injuries sustained by seniors living in their own home are generally available from hospital admission and emergency department data or through self report. The project team was unable to recommend a standardised incident report form for inclusion on this site. It should be noted however, that seniors reporting a fall, or in particular multiple falls, should be encouraged to keep a record of their falls. That is:
