Incident forms

Details of falls and injuries sustained by seniors living in their own home are generally available from hospital admission and emergency department data or through self report. The project team was unable to recommend a standardised incident report form for inclusion on this site.

It should be noted however, that seniors reporting a fall, or in particular multiple falls, should be encouraged to keep a record of their falls. That is:

  • the time and location of the fall/s,

  • the activity they were undertaking at the time,

  • how they were feeling at the time (eg dizzy, ill, distracted etc),

  • if there were any environmental causes, and

  • any other factor they feel may have contributed to the fall.

This additional information may help identify a pattern to their falls and can assist in the falls risk assessment process.
