Home < Australian Health Ministers’ Advisory Council (AHMAC) National Action Plan for improving the care of older people across the acute-aged care continuum, 2004-2008From hospital to home - Improving care outcomes for older people 2006–07 From hospital to home - Improving care outcomes for older people 2005-06 From hospital to home - Improving care outcomes for older people, (First) Annual Report, National Action Plan – Progress towards implementation 2004-05 The National Action Plan was developed by the Care of Older Australians Working Group, a subcommittee of the Australian Health Ministers’ Advisory Council (AHMAC), with support of its Clinical Reference Group. It was endorsed by all Australian Health Ministers in July 2004. The Plan contains a commitment that reports on progress under the Plan would be presented to Ministers at the end of each financial year and be made publicly available. This is the first report. The Australian Government and States and Territories have made progress over the year 2004-05 towards implementing a range of goals identified in the National Action Plan. Each jurisdiction is implementing the National Action Plan goals in the context of the particular structure, the availability and the need for health and aged care services in that jurisdiction.
