Home and Community Care provides a range
of home based services for children across the state with
acute, post acute and long term health care needs. One of
these services is Hospital in the Home, which currently provides
acute home treatment to children throughout metropolitan Melbourne.
Children remain under the medical care of hospital consultants.
Nursing and allied health care is mainly provided by hospital
staff, with minimal subcontracting of other service providers.
Children commonly managed through Hospital in the Home require
intravenous antibiotic therapy for conditions such as cystic
fibrosis, meningitis and pneumonia. Oximetry monitoring for
children with chronic lung disease or obstructive sleep apnea
is routinely done through the program as is ambulatory blood
pressure monitoring for children with renal disease.
Children with complex care needs are often initially managed
thouth the Hospital in the Home Program and then can be seamlessly
transferred to longer term services provided by the department
as required.

|
The Hospital in the Home program has adopted a multi-disciplinary
approach, utilising a "mixed-model" of service delivery. Patients
are referred by clinical units from the emergency department,
hospital wards, pre-admission/outpatient clinics. Direct medial
care is provided under a "shared-care" model, utilising hospital
medical staff and GP's. The nursing service and allied health
care is provided by employed personnel from within the hospital.
The program provides home-based acute health care for patients
with a range of conditions such as those requiring I.V. medications,
anticoagulation therapy, initiation of insulin and post surgical
wound care. |

The Hospital in the Home program at the Royal Women's Hospital
offers port-operative, maternity and neonatal patients the
opportunity to be treated at home where possible. Therapies
include intravenous antibiotics, wound care, trail of voiding,
administration of cytotoxic chemotherapy, oxygen therapy,
maternal bonding and gavage feeding.

This is a specialist HITH model, medical units being responsible
for medical care twenty four hours a day. The nursing service
is a mixed model with unit nurses delivering 45% of nursing
care and 55% being contracted out. Monash pharmacy compounds
drugs with some being delivered by Dandenong pharmacy. There
are four streams of patients: IV antibiotic program, cardiac
program, Low molecular weight heparin and a general program.
The largest program is the I.V. antibiotics using PICC lines
and ambulatory pumps when appropriate deliver Fragmin (LMWH)
to patients with DVTs or requiring anticoagulation pre and
post procedures. Provide other infusion treatments such as
methyl prednisolone. We give a small number of chemotherapy
treatments.

St Vincent's at Home incorporates HACC, PAC and HITH services
for clients of St Vincent's Hospital. The Hospital in the
Home program can provide nursing and physiotherapy to people
living within metropolitan Melbourne. Individuals experiencing
homelessness may stay in the Sister Francesca Healy Cottage
to receive HITH care. St Vincent's Hospital is currently introducing
GP involvement in the HITH program.

The Sandringham Hospital in the Home Unit provides a holistic
and individualised approach to patients who are suitable for
acute home based nursing care. The personalised care delivered
to the patient at home is provided by Sandringham HITH nursing
staff and is available for up to 2 visits a day, 7 days a
week. To those patients that reside outside of our borders
we will subcontract out the care. The region we generally
treat extends along the bay from St Kilda to Carrum with the
Princess Hwy as our Northern Border and Springvale Road as
our Eastern Border. HITH may be utilised pre-operatively,
following an in-patient stay, or may substitute a full ward
(hospital setting) admission by being admitted directly to
HITH via the Emergency Department. Our patients are reviewed
in clinic as required by their medical doctor and a HITH Coordinator.
Some of our common treatments include; Anticoagulation for
DVT, PE, AF and in the perioperative period; IV Antibiotics
for Cellulitis and Osteomyelitis; Acute drain and wound management
postoperatively.

A general program that will take all client types if the
HITH Admission criteria is met. Clients are usually transferred
to HITH from the acute wards or occasionally directly from
the Medical Centre.
After admission is organised by the HITH Co-ordinator the
continuing care is attended by the Stawell District Nurses
on behalf of HITH.
The most common client types are those requiring I.V antibiotics
or thrombolytic therapy for DVT's. |

The HITH Program at Werribee Mercy Hospital was established in 1996 and is staffed by an EFT of 2.4. We provide a service from 08.00am - 20.00pm with an after hours service provided by the ED and After Hours Co-ordinator. Referrals are accepted from within the hospital, through our ED department or those patients accepted by the Consultant Surgeons or Physicians of the hospital. Patients are reviewed regularly within the Ambulatory Care Unit by their medical doctor and a dedicated HITH Registered Nurse. We provide a service to our patients within a 30km radius of the hospital. Our core business is mainly cellulitis, management of anticoagulation therapy for DVT/PE/AF/MVR, I/V antibiotics, diabetes for stabilisation and management of drain tubes or complex wounds. |

Our Hospital in the Home Program is a mixed model with medical services being provided by community based GPs with admitting rights to the Hospital, or the specialist medical staff of the Hospital. Suitable patients are referred to the HITH Continuum of Care Nurse who assess and admits the patients to HITH. Nursing services are provided by the Hospital's District Nursing Service or by externally contracted district nursing services. The Program utilises both hospital and contracted services such as allied health, Pathology, Meals on Wheels and Home Help as required.
West Gippsland Healthcare Group provides a broad range of health services primarily to the community of the Baw Baw Shire which is situated approximately 100km east of Melbourne and covers 35,000 square km. Because of the large area serviced our district nurses spend approximately 33% of their time on the road, often in mountainous terrain and adverse weather conditions.
WGHG’s Hospital in the Home Program commenced in September 1996. From this time the program has gradually expanded with the target 2004-2005 being set at 2500 HITH days.
The main clinical treatments fall into the three broad groups of wound management, anti-coagulation and intravenous antibiotic therapy and post-op care. |

Western Health Hospital in the Home incorporates 3 campuses. – Western, Sunshine and Williamstown hospitals. The service covers an area from Melton, Sunbury to Glenroy, Williamstown and Werribee. Patients we treat range from the premature infants, children, midwifery and oncology patients to the acute general patients.
Treatment we provide is acute nursing service such as intravenous antibiotics, low molecular weight heparin and complex dressings. We also provide a P.E.G. (percutaneous endoscopic gastrostomy) service to Western patients in nursing homes. |

The Hospital in the Home program at the Wodonga Regional
Health Service is a program headed by a nursing coordinator
Referrals come from nursing staff, and medical staff. 60%
are transfers from hospital. 40% are directly admitted to
the program from general practitioner's rooms or the emergency
department. The program is staffed by dedicated HITH staff
from 0800-2130, Monday to Friday and 0800-1630 on weekends
and public holidays. Night coverage is attended by emergency
department staff.
The program is broad based involving care of medical and
surgical conditions for adults and children.
