Alfred @ Home has grown dramatically since it was established
in August of 1997, and is now one of the largest metropolitan
programs. Alfred @ Home staff deliver care to those patients
residing within the inner south east region of Bayside Health,
and subcontract nursing care to RDNS/District nurses to those
patients that reside outside of the region. The diversity
and acuity of the core speciality services at the Alfred has
provided the Alfred @ Home service with one of the highest
adult acuity in the state.
Alfred @ Home provides 24-hour on-call support and accepts
after-hour referrals from the Alfred Emergency Department
and General Practitioners. There are dedicated pharmacy resources
and a medical doctor committed to Alfred @ Home to assist
in providing a comprehensive service to achieve the desired
patient outcomes whilst under HITH. Patients are reviewed
regularly in clinic by their medical doctor and allocated
Alfred @ Home co-ordinator. Alfred @ Home also interfaces
with the Mobile Assessment Treatment and Rapid Assessment
Service to further assist in maintaining patient's care in
the community environment, thereby minimising unnecessary
ED presentations.
Some of the Alfred's more common DRG's include; osteomyelitis,
DVT, cellulitis, exacerbation of cystic fibrosis, warfarinisation
in the perioperative period, and acute wound/drain management
postoperatively. |

Patients at their Home Service (PATHS) is a generalist Hospital
in the Home program with a mixed general practitioner and
hospital referral base. PATHS offers a seven day per week
service with a twenty four hour on call services. Permanent
staff are skilled HITH nurses with physiotherapy, occupational
therapy and medical staff attached on a sessional basis. Common
treatments include: intravenous antibiotics, complex wound
care, anticoagulation therapy, diabetes management and post
surgical care eg draintube management. |

The A&RMC HITH model is mixed. A large component of HITH
activity (95%) is currently based on the principles of case
management. The treating medical unit refers the patient and
services are contracted from both internal and external service
providers. These services are purchased based on the patient's
individual care plan and may involve a multidisciplinary mix
of providers such as nursing, medical, allied health, and
support services.
An A&RMC HITH case manager co-ordinates this plan and ensures
that the outcomes are met in accordance with the patient,
carer and treating medical unit's expectations. A twenty-four
hour seven day a week on call service is offered and patient's
carers, service providers and the treating medical units can
contact the HITH Case manager at any time.
The case managers will formulate a discharge plan that will
be communicated to the patient's own G.P and the various service
providers that will be involved with the patient when discharged
from HITH.
The concept of home based acute care treatment is continuing
to gain acceptance both by health professionals, patients
and the wider community. The main clinical care modalities
that are organised for acute home care: Intravenous antibiotics/therapies/rehydration,
chemotherapy, complex wound management, post operative care,
enteral and parenteral feeding, respiratory treatments, anticoagulation,
early discharge programs in vascular, cardiac and orthopaedic
surgery.
This list is not finite. New technology, active research,
and quality data that indicate the safety and efficacy of
acute home care enhance the potential for new development.
This is further enhanced by the commitment of health care
administrators to the recognition of HITH as a core business
unit of the organisation. |

Ballarat Health Services - Base Hospital Hospital in the
Home program is a shared model of care. Medical care is provided
by both hospital staff and the patient's nominated general
practitioner. The clinical delivery of care is provided by
hospital and district nursing staff. A radius of 10 kms from
the hospital applies to patients to whom the hospital staff
deliver service, for patients who live outside this radius
but within the region, care is provided by the relevant District
Nursing Service. The hospital is a large, regional healthcare
facility providing a diverse range of care including neonatal
obstetric, specialty and general medicine, surgery and paediatrics.
Patients are admitted to the program from inpatient care,
emergency department, outpatients, pre-admission clinic or
metropolitan hospitals. Our hours of operation are 7am - 9.30pm
with overnight on call service provided by the District Nursing
Service.
The therapies we provide at home include
wound care, anticoagulant therapy, intravenous antibiotics,
cellulitis, antenatal care and a range of paediatric care. |

Program Description |

The Home Referral Service (HRS) offers an integrated model
of care that
includes HITH, Post Acute Care, District Nursing liaison
and Local
Government liaison services. The service aims to provide
a single point of
referral to assist with the transition of patients from acute
to community
based care.
Key areas of HITH activity include the management of infectious
diseases,
anticoagulation management, complex wound management, post
surgical care and
diabetes management. HITH patients are classified as inpatients
of The
Geelong Hospital and they remain under the care of the admitting
medical/surgical unit. All patient care is co-ordinated
by staff from the
Home Referral Service whilst the direct care component of
HITH is brokered
to Barwon Health and regional district nursing services.
This enables a
continuum of care for patients as they transition through
an episode of care
from acute to community services. Home Referral Service provides
a 24 hour
on-call service for HITH patients and HITH referrals. Paediatric
Home& Community Care (PHACC) offers
a similar model of care to all suitable
paediatric patients, with all direct care provided by Barwon
Health acute

Benalla Hospital in the Home Program is a part of the Home
Nursing Service. The three services, Hospital in the Home,
Palliative Care and District Nursing share common management
and location. Therefore the client moves seamlessly from one
service to another if required. Hospital in the home provides
hospital level acute care for people in their own homes from
a multidisciplinary approach with General Practitioners, Specialists,
nurses and allied health care workers combining together to
provide care. It is an extension of the Benalla and District
Memorial Hospital inpatient services. This service provides
therapies including intravenous antibiotics, extensive complicated
wound care, post operative follow-up as required, diabetes
education and management particularly for newly diagnosed
patient and acute pain management. |

Program Description |

Bendigo Health HITH is a mixed model - consultant
unit and acredited GP's with admitting and visiting rights.
The nursing staff = 1.64 FTE. It is a 24 hour service operating
7 days a week with emergency department backup and a 10 bed
capacity. The service extends to a 20km radius. GP follow
up reviews are conducted at home or in the surgery. Reviews
also take place in the out patient department. Treatments
provided include antibiotic therapy, hydration, pulse steroids,
anticoagulation therapy, wound management and monitoring.
Case types include: cellulitis, osteomyeltis, septicaemia,
wound dehiscence, pneumonia, DVT and MS. |

The Wellington Acute Care at Home program has a centrally
co-ordinated model of care. The service is no longer provided
through District Nursing. Care is provided by our own HITH
staff from a range of acute care settings and a broad experience
base. The majority of the program at this stage focuses on
the administration of intravenous antibiotics and low molecular
weight heparin. |

The Djerriwarrh Health Services (HITH) programme at Bacchus March receives referrals from both VMO's and LMO's. They also treat patients referred from metropolitan Melbourne and Ballarat hospitals. The services they offer include diabetes stabilization, intravenous antibiotics and complex wound care. The services are available for care 7 days a week from 8am-9pm and provide a 24 hr call service for their HITH clients. |

Goulburn Valley Health Shepparton provides HITH under a dual model of case management using either hospital medical officers or General Practitioners. The service operates 7 days a week between the hours of 8am - 8pm, with 24 hours on call care provided by GVH's District Nursing Service. GVH's has appointed a Hospital in the Home Coordinator who is based at Goulburn Valley Health main campus. The HITH Coordinators principal role is to identify clients from all specialties within GVH campus, who may be suitable for continued treatment under the HITH program. Referrals are also accepted and admitted to HITH from GP rooms. GVH has extended the reach of its HITH program by contracting our services to several outlying hospitals. |

Program Description |

The Mercy Hospital for Women's Special Care Nursery program
is called Premature and O2 dependant babies in the Hospiatl
in the Home Program. The program is designed to give parents
of babies in the Special Care Nursery the option of bringing
their child home to continue their care themselves shile still
remaining under the auspices of the hospital. This allows
the family to bond in their own environment with the security
of a visitng nurse and knowing that help is only a telephone
call away 24 hours a day. The baby is still considered an
inpatient and therefore able to return should the parents
or nurse deem it necessary. Most of the babies on the porgram
are those on oxygen therapy, gavage feeds or trying to establish
breast feeding.Parents take total care of the babies with
backup including emotional and pyschological support provided
as needed by the nursing staff. |

The Hospital In The Home Service provides home-based care to persons residing within a twenty-kilometre radius of Mildura Base Hospital. Highly skilled nursing staff are available from 0700 hours - 2200 hours seven days a week with an on-call service over night. Referrals are accepted from GPs, MBH Emergency Department as well as other centres. Medical care is provided by both VMOs, HMOs and GPs with support from Allied Health professionals including Physiotherapists, Occupational Therapists, Dieticians and Speech Pathologist.
The main treatments provided are intravenous antibiotics, anticoagulants, wound care and post-surgical management. |

Hospital in the Home at Northeast Health Wangaratta is one of many programs
managed in the Home Nursing and Support Services Unit. Other programs
include the District Nursing Service, Palliative Care, Post Acute Care,
Stoma therapy and Breast care. Hospital in the Home (HITH) has a coordinator
and two nurses who cover the program seven days a week. Local District
Nurses and Nursing Supervisors provide clinical care in the evenings and
after hours. We also sub contract external District Nursing Services in
smaller rural towns throughout the Hume Region to provide HITH care whilst
the patient remains admitted and managed under the NHW medical officer. Some
local General Practitioners in these rural towns also have HITH admission
rights and access the program as required. This fosters patient choice and
eliminates the need for the patient to travel to Wangaratta for admission.
Treatments include antibiotic therapy for Cellulitis, long term management
with PICC lines for Osteomylitis, Septic Arthritis, complex wound care,
anticoagulation therapy for DVT, PE and pre and post operative Clexane. |

PM@H incorporates HITH and HACC services for Peter Mac patients living within a 30km radius of the CBD. This acute nursing service is available seven days a week to patients in their homes, between the hours of 0800 and 1800. Patients requiring services outside our boundaries are outsourced to accredited agencies by the Community Liaison Nurse. Services include administration of cytotoxic chemotherapy, maintenance of central venous access devices, management of complex wounds and nursing care following chemotherapy, radiotherapy and surgery and referral to other services as and when required. Care is provided by Division 1 Registered nurses with specialised oncology knowledge. |

Portland and District Hospital's (PDH) "Hospital in the Home"
program is jointly coordinated by the four charge nurses of
our nursing departments North Ward, South Ward, Accident &
Emergency and District Nursing Service.
The clinical care is carried out in clients homes by the
district nurses making Hospital in the Home,Post Acute Care,
Palliative Care and Home Hursing Programs come together under
the District Nursing Service.
On call arrangements are made with PDH as the point of contact.
Heywood clients using Hospital in the Home use Heywood district
nurses for clinical care with PDH medical officers and management.
Treatments include post surgical dressings, IV antibiotics,
pre and post operative DVT care and other treatments as prescribed
by medical staff. |
