ACA Mission
To support Victorian health care providers as regional & international
health industry leaders in the development, trialing and evaluation
of ambulatory care initiatives involving the convergence of healthcare,
technologies and communications.
In accordance with current state and national ambulatory care reform
initiatives, the activities of the Centre are underpinned by the
following philosophical principles:
- People are the major health care resource, and as such they
require support and education in health promotion and illness
prevention, and empowerment to become a partner
in the management of their own health.
- People have the right to:
- access models of care which are patient centred,
- make informed choices about their health management
options, and
- retain responsibility for their health status.
In undertaking its work the Centre will:
- evaluate the effectiveness of innovative, ambulatory
care initiatives,
- foster ambulatory care delivery models based on a multi-disciplinary
team approach, which are coordinated, cooperative, longitudinal
and integrated. These models will incorporate case management
and clinical care protocols where appropriate,
- define best practice in patient-centred health care
through:
- the development of standards for ambulatory care,
- identification of key performance measures,
- benchmarking with similar services provided nationally
and internationally, and
- participation in ambulatory program reviews.
- identify and promote the knowledge base, skills and competencies
required by ambulatory health care professionals through:
- consultation with health care professionals and universities,
- provision of alternative clinical placement experience
for medical, nursing and allied health students,
- provision of research supervision in collaboration with
universities.
- improve equity in access to specialised services through
the evaluation of home health care enabling technologies such
as telehealth and remote monitoring, and
- provide a comprehensive ambulatory care resource for
health policy makers, health care professionals and the community.
