Aboriginal Health Promotion and Chronic Care (AHPACC) partnership initiative
Background
The Aboriginal Health Promotion and Chronic Care (AHPACC) partnership initiative supports Aboriginal community-controlled health organisations and community health services to work in partnership to improve health outcomes for Aboriginal people in Victoria with or at risk of chronic disease. There are currently eleven AHPACC partnership sites across Victoria. The Victorian Aboriginal Community Controlled Health Organisation (VACCHO) supports the local and statewide implementation of AHPACC.
AHPACC aims to contribute towards improving the length and quality of lives of Aboriginal people in Victoria with a particular focus on increasing access to comprehensive primary healthcare. AHPACC partnerships deliver services and programs to the Aboriginal community, as well as establishing new settings for services and programs and undertaking community engagement, workforce development, organisational change and building partnerships and inter-sectoral collaboration.
In 2010-11 a developmental review of AHPACC was conducted in order to strengthen the program and inform policy directions. For more information about the review process please view the bottom of this page.
Guidelines and Strategic Directions 2011-14
New guidelines for the AHPACC initiative were published in April 2011 and are effective 1 July 2011 to 30 June 2014. The guidelines relate directly to the findings of the 2010-11 AHPACC Review. They define six types of activities and five service delivery models for AHPACC. The guidelines also include new reporting requirements and a new program logic model that specifies the program's intended outcomes and key external influences.For more information please view the following document:
Reporting Requirements
New reporting requirements have been introduced for AHPACC effective 1 July 2011. Details of the reporting requirements are included in the
AHPACC Guidelines and Strategic Directions 2011-14. A timeline for reporting in 2011-12 is below.

AHPACC Continuous Quality Improvement (CQI) Tool
The AHPACC CQI Tool was developed out of the success factors identified in the 2010-11 AHPACC Review. The tool is intended to support planning and prioritising of activity by AHPACC partnerships; its completion is also a requirement of funding in 2011-14. It may also assist other organisations and partnerships implementing Aboriginal health programs.
Some minor changes were made to the tool in March 2012 and the current version is available below.

AHPACC Industry Advisor
In order to support AHPACC agencies and partnerships develop their work in line with the new Guidelines, a range of new strategies have been put in place by the Aboriginal Health Branch in partnership with VACCHO, and one of these is the AHAPCC Industry Advisor.
The document below outlines the types of resources that the new Industry Advisor can provide, as well as identifying support and resources available from the Department of Health and VACCHO during 2011-12.

AHPACC Managers Network
The Department of Health convenes a quarterly meeting of AHPACC Managers. Meetings are held at 50 Lonsdale Street, Melbourne and videoconferencing facilities are made available in Shepparton, Mildura, Warrnambool, Bairnsdale, Wodonga, Swan Hill and Dandenong.

AHPACC Strategic Project Grants
Fifteen grants were awarded in May 2011 to successful applicants who are pursuing a range of projects to implement the strategic directions resulting from the 2010-11 AHPACC review in their local partnerships.

Findings from the 2010-11 AHPACC Review
A developmental review in 2010-11 found that the AHPACC initiative has made some significant achievements in the areas of service innovation, community engagement, intersectoral collaboration and organisational change for cultural responsiveness. The findings indicate that AHPACC is contributing to improving the length and quality of Aboriginal Victorians' lives. Opportunities to strengthen the initiative include increasing workers' role clarity, clarifying reporting, enhancing integration and connection with other programs, and ensuring access to support and networking opportunities across all sectors.
