Overview
Primary Care Partnerships (PCPs) are made up of a diverse range of member agencies. All PCPs include hospitals, community health, local government and divisions of general practice as core members of the partnerships. Other types of agencies such as area mental health, drug treatment and disability services are also members of PCPs. The partners can also be specific to local issues and needs. For example, some PCPs have engaged with the police, schools and community groups.

Partnership assessment tools
Primary Care Partnerships are required to adopt a quality improvement approach to assess partnership strengths and weaknesses using the VicHealth or the Centre for the Advancement of Collaborative Strategies in Health partnerships evaluation tool.

- Information from Primary Care Partnerships

- The Australian Journal of Primary Health is a peer-reviewed journal designed to publish articles on a range of issues influencing community health services and primary health care. Edition volume 15, number 3, 2009, Part 1, Partnerships in Care and Primary Health illustrates the key issues in developing and sustaining partnerships across the primary health care sector. Available through CSIRO Publishing.
- The United Kingdom, National Health Service (NHS), Institute for Innovation and Improvement have developed a guide titled ‘Developing a Compact: A brief guide for Clinical Commissioners’. The first section of the document focuses on partnership development.
- The Victorian Council of Social Service (VCOSS) have produced three ‘Partnership in Practice Guides. These guides provide information, tools and resources that examine the three stages of partnering: preparing to partner; commencing the partnership; and, sustaining the partnership.

Integrating health and social care in Torbay – Improving care for Mrs Smith - UK King's Fund
This paper sets out how one particular area – Torbay – created an integrated care system that aimed to improve care for ‘Mrs Smith’, a fictitious user of health and social care services.The case study provides the background to the development of integrated care, the setting up of a pilot team, the implementation of an integrated management structure, the establishment of Torbay Care Trust (which is a fully integrated NHS organisation responsible for commissioning and providing community health and social care services) and assesses the impact of the integrated system on the performance of the Torbay health and social care economy.
