Evaluation has included:
PCP Strategy: Evaluation of the PCP StrategyThe Department of Human Services engaged the Australian Institute of Primary Care at La Trobe University to conduct a series of studies to evaluate the PCP Strategy.
The recommendations from this report have largely informed the Primary Care Partnerships Strategic Directions 2004-2006 which outlines the key achievements of the PCP Strategy, the success factors and ongoing challenges. Please also see attached a summary of the response by DHS to the specific recommendations in the final report of the evaluation of PCP Strategy.
Service Coordination: Analysis of the impacts of Service CoordinationResearch completed by KPMG looked at the impact of Service Coordination on five community health services and three local government HACC providers. It found that when successfully implemented, Service Coordination delivers benefits to agencies, practitioners and consumers. Integrated Chronic Disease Management: Lessons LearnedStudies strongly suggest that people with chronic disease have a better quality of life, experience fewer complications and reduce their overall use of health care resources if they programs are tailored to their needs and health services are coordinated where multiple providers are involved. The following document details the work and improvements that have been delivered through a range of integrated chronic disease management initiatives delivered in the primary care setting including those delivered through PCPsICDM Status Report 2008-10: Following an evidence based review, five IDM pilot projects were funded for three years in 2001 under the Department of Human Services (Victoria) IDM Initiative.
