Overview

Evaluation of the Primary Care Partnership (PCP) Strategy has been conducted to provide government departments, PCPs and the sector with information to maximise learning from the reform, to provide a basis for refinement of policy and service development and to determine whether, and to what extent, the PCP Strategy has achieved its objectives.

Evaluation has included:

  • AIPC reports on evaluation of the PCP Strategy
  • KPMG project to assess the impact of Service Coordination
  • Maps of program logic for strategic initiatives of the PCP Strategy
  • Information resource about the evaluation of the PCP Strategy.

PCP Strategy: Evaluation of the PCP Strategy

The Department of Human Services engaged the Australian Institute of Primary Care at La Trobe University to conduct a series of studies to evaluate the PCP Strategy.

  • The most recent report An evaluation of the PCP Strategy, October 2005 presents the 2005 evaluation of the activities of PCPs, including the effects on agencies and consumers, for 2005.

    Particular attention is given to providing comparative analyses from data collected in 2002 and 2003. Overall, the findings show that, in the last five years, the PCP Strategy has brought about significant integration within the primary health care system and this has resulted in improved coordination of services and more positive experiences for consumers with the health system.

In presenting the findings, consideration is given to the previous baseline and interim evaluation reports (which can be accessed at the AIPC website, refinement of the PCP Strategy in 2002, & the current implementation & impact of the PCP Strategy.

The recommendations from this report have largely informed the Primary Care Partnerships Strategic Directions 2004-2006 which outlines the key achievements of the PCP Strategy, the success factors and ongoing challenges.

Service Coordination: Analysis of the impacts of Service Coordination

Research completed by KPMG looked at the impact of Service Coordination on five community health services and three local government HACC providers. It found that when successfully implemented, Service Coordination delivers benefits to agencies, practitioners and consumers. Integrated Chronic Disease Management: Lessons Learned

Studies strongly suggest that people with chronic disease have a better quality of life, experience fewer complications and reduce their overall use of health care resources if they programs are tailored to their needs and health services are coordinated where multiple providers are involved.

The following document details the work and improvements that have been delivered through a range of integrated chronic disease management initiatives delivered in the primary care setting including those delivered through PCPs

Following an evidence based review, five IDM pilot projects were funded for three years in 2001 under the Department of Human Services (Victoria) IDM Initiative.
